New Patient Forms

After you have made your appointment for either Naturopathic Medicine and/or Acupuncture, create your patient portal login and password. In the patient portal, you will be able to complete your new patient forms including health history and associated clinic procedure forms. If you would prefer to have them sent in the mail, just let us know via email or calling the clinic. Dr. Becker will need to have all of your paperwork completed ahead of time to be able to prepare for your appointment. Also, remember to share any pertinent lab work and current medications and supplements.  This information can be uploaded into the patient portal as well. To access the patient portal, click below.
